DCB for calcified nodules: key insights from the ALLIGATOR study

How does nodular calcification impact the outcomes of drug-coated balloon (DCB) angioplasty for femoropopliteal lesions? In this exclusive discussion, Dr. Naoki Hayakawa, interviewed by Dr. Masahiko Fujihara, shares key findings from the ALLIGATOR study—a detailed analysis of clinical outcomes in patients with nodular calcifications undergoing DCB treatment.

He explains the study design, evaluation criteria, and patient data, before presenting the main results. The study highlights that nodular calcifications are independently associated with post-DCB outcomes. However, as a non-randomised, retrospective study, it comes with limitations—data was collected in the early days of DCB use in Japan, and atherectomy devices were not yet incorporated into treatment strategies.

With advancements in techniques, could DCB outcomes be further improved? Larger, up-to-date studies are needed to refine our understanding.
